This is a confirmed **LLM01 Prompt Injection** finding (**P0**): both the user query and retrieved chunks are untrusted input, and retrieved documents may contain instructions that override the intended prompt.

Also review:

- **LLM08 Vector & Embedding Weaknesses**: sanitize indexed text and enforce tenant namespace isolation.
- **LLM02 Sensitive Information Disclosure**: remove credentials/PII before adding chunks to context.
- **LLM05 Improper Output Handling**: sanitize model output before using it in HTML, SQL, shell commands, or redirects.
- **LLM10 Unbounded Consumption**: set `max_tokens`, rate-limit requests, and cap retrieval/agent iterations.

Additionally, reject oversized or malformed inputs, strip or quarantine suspicious instruction-like content where appropriate, retain document provenance, validate tenant access before retrieval, and apply output validation before downstream use. Assume retrieved chunks are untrusted even when they originate from an internal repository.
